Situation

Jonathan is 66 years old and was diagnosed with Primary Progressive MS in 2007. He lives alone and privately funds all of his care provision.

Background

  • As Jonathan lives in a rural area, care facilities are limited. He therefore has to privately fund all his social and informal care. However this is no longer a financially viable option.
  • He is dependent on someone providing all aspects of health and social care in order to remain at home.
  • He has bladder issues, swelling to the joints and limbs, and has been reliant on a wheelchair since 2013.

AMSC Input and recommendations

  • The AMSC has made an urgent referral to the Adult Social Care team for a financial assessment and general assessment of needs. They also completed an assessment to ascertain Jonathan's ongoing care needs.
  • A referral was made to the Occupational Therapist team to assess the home environment.
  • Provided training and advice for carers on bowel management, medication and diet .
  • Involved a physio to provide carers training in relevant stretches and positioning.
  • Established strong patient relationship and regular contact.

Outcomes

  • Implemented a joined up care approach through close working with health care professionals and adult social care services.
  • Avoidance of unnecessary hospital admissions.
  • More frequent reviews for Jonathan.
  • Safeguarding and advocacy to ensure Jonathan can remain in his own home.

The role is definitely needed, it is so helpful to have a point of reference if you have a concern, a question or a thought about an aspect of care, like funding for example. The role is not so much nursing, but getting your life sorted so that you can relax a little bit.
